Health Promotion Nurses/HCAs for Community NHS Health ChecksWe are the leading provider of Community NHS Health Check events, targeting disadvantaged and hard-to-reach users.

We are now recruiting for Nurses and HCAs with health promotion knowledge to join this nurse-led service to deliver the checks and give appropriate lifestyle advice.

Job Specification

  • Conduct health checks, finger prick sampling, BP readings, body measurements in community settings
  • Communicating patients' their risk of developing a chronic condition
  • Help patients to understand how lifestyle changes could reduce their health risks
  • Encourage patients to commit to lifestyle changes and accept referrals to lifestyle programmes
  • Refer patients as necessary to GP, walk-in centres or A&E
  • Comply with all procedures to ensure patients receive a quality service in a non-clinical setting
  • Collaborate with consultants, psychologists, public health leads, commissioners and other nurses to enhance the service
  • Contact patients and GP practices in the future for audit and research purposes
  • Understand issues of patient confidentiality

Experience

  • Registered nurse or Healthcare Assistant
  • Passionate interest in helping patients to improve their health through lifestyle change
  • Worked on community health check or screening programmes or enthusiastic about learning about this
  • Good understanding of the risk factors that contribute to chronic conditions, (e.g. cardiovascular disease, COPD, diabetes), and how risk can be reduced and conditions managed through lifestyle change
  • Provided lifestyle advice to patients with chronic conditions e.g. smoking cessation, weight loss
  • Positive, calm, detailed, thorough and reliable with empathetic listening and communication skills
  • Awareness of behaviour change techniques
